Dear graduate from the Augustana Class of 1999, 2004, or 2007: 

Welcome to the alumni survey page for the Classes of 1999, 2004 and 2007.  Thank you for taking the time to complete this survey.  Your perspective on Augustana and details of your post-graduate activities are extremely important to help measure the effectiveness of programs.

Augustana Alumni survey - Informed Consent Statement

Purpose and Description of this study

This is an alumni survey of the Augustana graduation classes of 1999, 2004, and 2007 being conducted as part of a joint research study by Augustana College, Allegheny College, College of Wooster and Washington College.  The purpose of the study is to investigate the impacts our educational programs are having on the personal and professional lives of alumni, with a particular focus on the impacts of senior capstone type experiences.  Questions will ask you to evaluate your undergraduate experiences, including your involvement and satisfaction with programs and their impact on your academic development, your current involvement with Augustana, and your post-graduation educational and career activities.   Ultimately, we hope a benefit of the study will be to improve our academic programs.

The survey instrument is the Alumni Survey of the Higher Education Data Sharing (HEDS) consortium, with supplemental questions concerning senior capstone experiences.

The survey takes about 20 minutes to complete.

Only the Augustana Institutional Research Office will have access to your responses with any personally identifiable information, and your responses will be kept secure and in professional confidence within the Institutional Research Office For research purposes, responses may be connected to selected data from student records such as majors, co-curricular activities and GPA.  After removing all personal identifiers, the for the alumni at the four colleges participating in the study will be merged and shared within their respective institutional research offices for purposes of statistical analysis and reporting. All reports will be at the level of statistically aggregated data. 

IPod Touch Raffle


In appreciation for your participation, Augustana will hold a drawing for an Apple 32GB iPod Touch among the Augustana participants.  According to Apple, this $300 iPod holds up to 7,000 songs or 40 hours of video.


Voluntary Nature


Participation in this survey is voluntary and participating or not participating will not affect your relationship to Augustana.  If you prefer not to respond to any question you may leave it blank.

There are no anticipated risks to you from participating in this study.
