Participation in Varsity Sports

Each student athlete is required to have a physical examination utilizing this athletic physical form prior to participation in any intercollegiate sport. Students will not be allowed to participate without this physical and the completed forms. The final decision on athletic participation status will remain the responsibility of Augustana's team physician upon thorough review of the athlete's medical information. A student athlete who doesn't return a complete pre-participation physical form prior to the onset of athletic participation will be responsible for scheduling and payment of a physical examination either from our team physician or a medical physician of your choice.

Athletes must fill out and return the forms to below prior to any athletic participation.


Insurance

Injuries occur and we attempt to provide our athletes with the very best possible care. Medical bills may be incurred when an athlete is treated for an injury


Medical insurance is a requirement for intercollegiate athletics participation. There are three insurance options that may be used individually or in combination with each other.


• Your personal family policy that you receive through your employment or through personal acquisition will always serve as your primary policy.


• The Augustana Student Accident and Sickness Insurance Plan is available through students' tuition bills.


• The Athletic Accident Insurance policy through Gallagher Koster. is provided for students by Augustana through their participation in intercollegiate sports. However the $500 deductible is the responsibility of the athlete. This deductible may be picked up by your personal family policy, the Augustana Student Accident and Sickness Plan or out-of-pocket.

Claims

All medical bills incurred as the result of an injury in intercollegiate sports will be sent to your personal family insurance provider if they are a member of the physician's approved provider list. If not, the bills will be sent directly to the student or their parents.


Any bills sent to students or their parents should be submitted to all insurance policy providers


Contact the Sports Medicine Department at (309)794-7573 if you have further questions regarding unpaid bills.


Please note that if your personal family policy is through a Health Maintenance Organization or a Preferred Provider Organization you must follow the proper procedures required by your plan. This is especially important if your plan requires pre-authorization for medical treatment if the injury occurs outside your personal family plan's service area.


Avoid injuries altogether by properly preparing for your athletic participation.
