How to document your vaccination status

Augustana's online Vaccine Status Survey has closed as of Sept. 3, 2021. However, students and employees who have not registered their status with the college still are required to do so.

Unregistered students should go in person to the Dean of Students office in Founders Hall, room 109, to present their documentation. Employees should contact Kirk Anderson in the Business Office.

Alternatively, students and employees can present documentation at the Augustana Convenient Care Clinic at their scheduled testing times to verify vaccination. Those who do so will not be required to test.

Those who have not completed the vaccine survey or provided documentation are considered unvaccinated and will be required to test weekly until vaccination documentation is provided.

Illinois and state vaccination records

Students and employees vaccinated in Illinois can go to the Illinois Resident Immunization Portal and follow the instructions to access their vaccination records. 

The college will accept this documentation. Students should download these records and share them with the Dean of Students.

Students vaccinated outside of Illinois should see their state's public health department  for more information.
