Enter through right side parking lot door of Centennial Hall to ensure physical distancing\; the lobby will be exit-only.\n\nBefore you arrive\n&nbsp\;\nYou must&nbsp\;print and fill out the medical consent form\, and bring a photocopy of your insurance card (both sides) and a photo ID. Paper forms will not be available the day of the clinic.\n\nIf you have any of these insurance providers\, your shot is covered:&nbsp\;\n\nBlue Cross Blue Shield of Illinois PPO\nHealth Alliance\nGallager\nHumana (Medicare)\nIllinicare (Medicaid)\nMedicare Part B\nMeridan (Medicaid)\nUnited Health Care\n\nCall the number on your insurance card to make sure the Rock Island County Health Department is in-network and the flu vaccine is covered\, especially if you have out-of-state insurance.\n\nIf you don't have insurance\, or the vaccine is not covered on your policy\, the cost is $35. Bring cash\, check\, or debit/credit card.\n
