DESCRIPTION:A flu vaccination clinic for all college employees will be offered from 11 a.m.-2 p.m.\n\nYou must&nbsp\;print and fill out a medical consent form\, and bring a photocopy of your insurance card (both sides) and a photo ID. Paper forms will not be available the day of the clinic.\n\nForm&nbsp\;for employees on the college health plan\n\nForm for employees using other insurance\n\nIf you have any of these insurance providers\, your shot is covered:&nbsp\;\n\nBlue Cross Blue Shield of Illinois PPO\nHealth Alliance\nGallager\nHumana (Medicare)\nIllinicare (Medicaid)\nMedicare Part B\nMeridan (Medicaid)\nUnited Health Care\n\nCall the number on your insurance card to make sure the Rock Island County Health Department is in-network and the flu vaccine is covered\, especially if you have out-of-state insurance.\n\nIf you don't have insurance\, or the vaccine is not covered on your policy\, the cost is $35. Bring cash\, check\, or debit/credit card.\n
