DESCRIPTION:The Rock Island County Health Department will offer a free flu vaccine clinic for students from 9 a.m.-2 p.m.\n\nNo appointment is necessary. Enter through the front door of Centennial Hall.\n\nYou must print and fill out the medical consent form\, and bring a photocopy of your insurance card (both sides) and a photo ID. Paper forms will not be available the day of the clinic.\n\nIf you have any of these insurance providers\, your shot is covered:\n\nBlue Cross Blue Shield PPO\nIllinicare\nMeridian\nUMR\nUnited Health Care\nYouthcare\n\nCall the number on your insurance card to make sure the Rock Island County Health Department is in-network and the flu vaccine is covered\, especially if you have out-of-state insurance.\n\nIf you don't have insurance\, or the vaccine is not covered on your policy\, the cost is $35. Bring cash\, check\, or debit/credit card.\n
