DESCRIPTION:The Rock Island County Health Department will be offering flu shots for employees and students on Friday\, Oct. 18\, in Centennial Hall from 9 a.m.-2 p.m. Please enter through the back door of the building and exit through the front. This is a walk-in clinic and no appointment is necessary.Individuals must bring the following items to the clinic:A copy of your insurance card (front and back)Completed consent form. Consent forms will be available at the clinic as well.The consent form also includes a list of accepted insurance programs. It is recommended that you call the phone number on your insurance card to make sure the Rock Island County Health Department is in-network and that the flu vaccine is covered\, especially if you have out-of-state insurance.If you do not have insurance\, or your insurance is not accepted\, the cost is $66 for the shot payable by cash or check.
