Student emergency contact and health information
Augustana College requests that you provide important health information to the college so we may best serve your medical and health needs.
Please provide this information regarding any current or chronic health concerns you may have by using Arches student portal.
Also be aware that Augustana College maintains a counseling staff that is available at no cost to you as a student. All records maintained in the Student Counseling Services are considered private and confidential, and are not shared with any person or entity within the Augustana community, or outside of our community, without your consent. These records may also be shared when there is a legal requirement to do so, such as the issuance of a subpoena.
As a student, you have the option to designate a person to whom Augustana College may disclose private mental health information. Augustana College will contact this designated person only if counseling staff determines that you pose a clear danger to yourself or to others, or to protect you or another person against a clear, imminent risk of serious mental or physical injury, disease, or death. You are not required to designate a contact.
Should you choose to designate a mental health contact person, please provide your information by using the Arches student portal. Your contact may be anyone over the age of 18. You may modify your designation at any time in the Dean of Student's office. Please note that Augustana College may contact your parent, legal guardian, spouse and/or other emergency contact as required or permitted by law without express written consent.
