Documentation guidelines

All students requesting accommodations at Augustana College must complete the Accommodations Request Form and provide one form of documentation of disability(s).

Depending on the type of disability/accommodation, different documentation may be required. Note that documentation generally must have been provided within the last three years to be accepted. Please call to request an exception. Those receiving accommodations will receive confirmation within two weeks of submission.  

Academic accommodations

Students who wish to receive accommodations in the classroom should submit their most recent Individualized Educational Plan (IEP), 504 Plan, or the Documentation of Disability form completed by a licensed professional. Documentation must be provided in the last three years.

Housing accommodations 

For most housing accommodations for students living in the residence halls, we accept the Documentation of Disability Form completed by a licensed professional. There are additional documentation requirements for emotional support animals. 

Upload documentation with the Accommodations Request Form or send documentation through email, or fax to 309-794-7152, attn: Michelle Mason, or by mail to Michelle Mason, 639 38th St., Rock Island, Ill. 61201. For new students, requests for housing accommodations are due by June 30.

Other accommodations

For any other physical accommodations at Augustana College, please submit the Documentation of Disability form completed by a licensed professional.

Documentation of Disability Form

If you do not have an IEP or 504 Plan, you may submit the Documentation of Disability form filled out by a licensed professional or have the licensed professional send us the documentation letter on a letterhead and signed by the licensed professional containing the following information: 

Diagnosis or diagnoses
Date of diagnosis or diagnoses
Date this letter was written
Date of last contact with the student
Detailed explanation as to how the disability or disabilities affect the student in an educational and residential setting. 
Detailed explanation as to which accommodations must be provided by Augustana College. 
Any other comments that may be helpful.
